it will be combination of planets that make someone narcissist.
saturn or his signs should be weak , as he must not have patience to think from different point of you . also can’t endure or bear situation easily.
mercury or his signs should be well placed, strong and afflicted for that person to think about only himself.
he will be narcissist with above both placements.
but for him to be psychopath he must also have moon afflicted.
for him to be serial killer, with above placement, his mars also should be strong.
